Register as a patient

We welcome new patients. You can register in person at the surgery from Monday to Friday, using the registration form or use one of the online links below. All patients will be asked to present proof of address of a bank statement, tenancy agreement or a utility bill within the late three months period. We strongly recommend all newly-registered patients have a basic medical examination, which can be booked with our healthcare assistants.

Online registration

To register online please click on either of the following links Online Registration 1 (New) or  Online Registration 2 (Old) and answer all the questions, eventually we will cease the older version. We will require you to complete a Zero Tolerance policy form and supply us with proof of ID and address, which you can email to us or bring into the practice for your first visit. Proof of ID will need to be a photo of a valid passport, driving licence or identity card along with a photograph of yourself. Proof of address will need to be a document within the last 3 months which proves your address, such as a bank or utility statement. Additionally if you are out of the main N15 area, we will require you to complete an out of area registration form as well, which can be found below. We endeavour to complete online registrations within two working days of submission

Registration Area
We currently take patients from within our catchment area. However, please be aware that if you are not in our inner region catchment area you will not be eligible for home visits.

Please be sure to tell us if you change your address or contact details. If you are moving to another surgery and wish to stay here please contact us so we can advise if this will possible.
